MacDQoL – Macular Dependent Quality of Life

To use this questionnaire you will need a licence from HPR Ltd. By obtaining a licensed questionnaire, you can be assured that you are using the most recent version that is appropriate for your study. This questionnaire is an individualised measure of the impact of macular disease on quality of life. It is suitable for adults with late-onset macular degeneration and those with other forms of macular disease.
